This Statement of Privacy applies to Wildflower Health’s health information service platform (the “Service Platform”), including the Due Date Plus mobile application and various web pages operated by Wildflower Health, Inc. (“Wildflower Health”).  This policy governs data collection and usage. By using the Wildflower Health Service Platform, you consent to the data practices described in this statement.

Collection of your Personal Information

Wildflower Health may collect personally identifiable information, such as your name, email address, health plan or individually identifiable health information. We may gather additional personal or non-personal information in the future.

Information about your computer hardware and software may be automatically collected by Wildflower Health. This information can include: your IP address, browser type, domain names, access times and referring website addresses. This information is used for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the Wildflower Health website.

Wildflower Health encourages you to review the privacy statements of websites you choose to link to from Wildflower Health so that you can understand how those websites collect, use and share your information. Wildflower Health is not responsible for the privacy statements or other content on websites outside of the Wildflower Health website.

Use of your Personal Information

Wildflower Health collects and uses your personal information to operate the Service Platform. When registering with Wildflower Health and using the Service Platform, we will ask you to provide us with your name, e-mail address, and monitored health data for which you are registering (e.g., your due date, characteristics of your pregnancy, your medical history and your health habits). As part of the services you receive from us, you may provide monitored health data, through self-reporting (e.g., pregnancy weight gain). In addition, you may submit additional information and comments through the Service Platform. It is always your choice whether or not to provide us with such information. If you choose not to provide the requested information you may not be able to use and receive certain benefits, features or functions of the Service Platform. Whenever you engage with our Service Platform, Wildflower Health also receives and records information on our server logs from your browser, including your IP address, cookie information, and the pages you request, and relates it to the personal information you provide.

Wildflower Health may also use your personally identifiable information to inform you of other products or services available from Wildflower Health and its affiliates. Wildflower Health may also contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.

Wildflower Health does not sell, rent or lease its customer lists to third parties.

Wildflower Health may share data with trusted partners to help perform statistical analysis, send you email or postal mail, or provide customer support. All such third parties are prohibited from using your personal information except to provide these services to Wildflower Health, and they are required to maintain the confidentiality of your information.

Wildflower Health may keep track of the websites and pages our users visit within Wildflower Health, in order to determine what Wildflower Health services are the most popular. This data is used to deliver customized content within Wildflower Health to customers whose behavior indicates that they are interested in a particular subject area.
